What is an LOA?
A letter of authorization: your HMO's approval for a consultation or a test. Request it online before your visit and the desk answers by the cut-off time.
What if my LOA does not cover everything?
The statement shows what your HMO pays and what is left for you. The difference is paid at the cashier.
Can I use my HMO for dental?
Only if your plan includes dental. Ask your HMO, or request an LOA and the desk will tell you what it covers.
